Interaxus, I have no idea how you found this - for I certainly looked - but it looks to me like a copy of the one which disappeared from Oxford University’s website when they did their reorganisation a few years ago. Many, many thanks for this.

I used mutool to split the original two-page pdf scan into a single- page pdf, as described here.
